Pressure Sores – What Are Pressure Ulcers and Bedsores?

A pressure sore is a portion of necrotic tissue that can appear on the skin, subcutaneous tissue, or mucosa. These are considered secondary skin lesions. Pressure sores or bedsores generally occur when there is constant and excessive pressure on the skin or tissues in any part of the body.

They are considered a consequence of remaining in bed for long periods or sitting due to immobility, reduced mobility, incontinence, or any physical or health limitation. Certain areas of the body have a higher risk of developing pressure sores, such as the gluteal region (sacrum, ischium, and coccyx 50%), followed by the heels, which bear the full weight of the legs (35%), the back of the head and occipital region (10%), and the shoulders (5%).

Pressure Ulcer and Bed Sore Grading

  • Grade 1: The skin is intact but begins to redden and does not fade when pressed. Changes in skin temperature may also occur, along with pain or a tingling sensation in the area.
  • Grade 2: The skin begins to lose thickness affecting the epidermis, dermis, or both, with superficial wounds that may appear as blisters or shallow craters.
  • Grade 3: The skin has lost its full thickness, and subcutaneous tissue necrosis may extend downwards, resembling a crater in the skin, though not affecting the lateral area where pressure is applied.
  • Grade 4: The skin has lost all its thickness with extensive destruction. There is necrosis of the tissue that may even affect muscle, tendons, or bone.

Treatment should begin as soon as a pressure sore is detected at grade 1 or 2 to treat the lesion early and ensure effective recovery.

Generally, treatment for pressure sores is highly variable and depends on the extent, depth, and cause of the injury. Therapeutic treatment should be applied if the sores already exist or preventive measures should be taken to avoid their development.   • Position changes: For people who need to remain in bed or seated for long periods due to physical or health conditions, changing position frequently is vital. Moving the body at regular intervals is one of the best ways to prevent the formation of pressure sores in grades 1 and 2. Position changes can vary from every 15 minutes to every 2 hours, depending on the risk.
  • Use of heel protectors, boots, and talon pads: These support surfaces help you lie down or sit in a way that protects vulnerable skin and avoids direct contact with the bed or chairs.
